Rental agreement

The equipment on the rental invoice has been received in good condition along with an operator’s manual and is accepted by Lessee. The rental period shall begin on and include the date of either delivery or pickup of the equipment to or by the Lessee or their agent. The Lessee shall pay rental for the entire rental period on each article of equipment prior to or at the beginning of each rental period. The Lessee agrees to not remove, alter, disfigure, or cover up any part of the equipment. The Lessee shall see that the equipment is not used carelessly or in a needlessly rough manner. Lessee shall at his/her own expense maintain the equipment and its appurtenances in good repair and operative condition as indicated in the owner’s manual. Lessee agrees to grease all grease points at least every eight hours of operation and agrees to check all fluid levels at least every 4 hours of operation and prior to and after each use. The Lessee shall indemnify the Lessor against all loss and damage to equipment during the rental period. Lessee assumes all risk and liability arising from Lessee’s possession, use and operation of each unit of equipment from the moment of delivery to Lessee to the moment of return to Lessor and agrees to indemnify and hold Lessor harmless from any and all of the following, whether the same be actual or alleged. Lessee takes/assumes All loss, damage, claims, suits, taxes, liens, penalties, fines, liability and expenses arising or incurred because of such possession, use and operation of equipment including but not limited to damages for injuries or death to persons or injury or destruction of property, claims and liens for storage, labor and material and all loss of and damage to equipment. Lessee accepts the equipment as used or second hand, unless otherwise noted on the rental contract. The Lessor shall have the right at any time to enter the premises occupied by the equipment and shall be given free access thereto and afforded necessary facilities for the purpose of inspection. Lessee shall provide and pay for all risk insurance against physical loss or damage to units in an amount equal to the full insurable value of the equipment. Such policies shall name Lessor and its assigns as loss payee as their interest may appear. Lessee shall provide and pay for public liability insurance including contractual liability for bodily injury and property damage caused by or arising out of Lessee’s possession, use, and operation of units. The minimum limits for such insurance shall be not less than five hundred thousand dollars.

I agree to additional charges on my credit card if the machine is returned damaged, past the due date and time, over allowed hour usage, or not full of fuel. I agree in the event of loss or insurable damages that I am responsible for a $2,000 deductible and authorize that to charge on my credit card on file
